• Manage daily workload allocation and supervise junior staff to ensure adherence to departmental policies.
• Provide comprehensive psychosocial assessments and care planning for vulnerable clients.
• Coordinate with government and charitable agencies and advocate for patient rights and long-term care needs.
• Facilitate safe discharge planning, crisis interventions, and psychoeducation.
• Maintain high standards of clinical documentation and confidentiality; ensure compliance with safety, infection control, and quality assurance protocols.
• Actively participate in training, professional development, and multidisciplinary collaboration to enhance service delivery and maintain regulatory standards.

Technical & Professional Competencies

• Produce comprehensive assessments of client needs, formulate social care plans, and identify resources to meet individual needs.
• Coordinate the delivery of social care plans and liaise with government or charity agencies when appropriate.
• Conduct initial person-centered psychosocial assessments and develop care strategies with the service user, caregiver, and multidisciplinary team; update patient files as required.
• Advocate for patients to ensure their expressed wishes are central to care planning and long-term needs.
• Consider legal issues for patients and liaise with admitting authorities when necessary; assist with discharge planning.
• Engage with other agencies and professionals to provide services and facilitate access to resources; support patients and families in finding appropriate resources.
• Maintain high standards of clinical record keeping (electronic data entry, reports) and uphold professional codes of conduct.
• Provide crisis interventions and participate in complex discharge planning with the multidisciplinary team.
• Participate in multidisciplinary team meetings and provide psychoeducation and direct therapeutic interventions as warranted.
• Promote anti-discriminatory practice and facilitate multidisciplinary collaboration.

Qualifications

• Bachelor’s in Social Work and Master’s degree in Social Work.
• Two years of experience after postgraduate qualification.
